The standard flat monthly membership. In-office and virtual care, with direct access to your physician in between.

One clear monthly membership
No tiers named after how much we think you'll pay. A simple flat rate for adults, a lower rate for children — with everything that matters included from day one.
HSA-eligible. As of January 2026 you can pay your membership with pre-tax dollars straight from your Health Savings Account.

The VIP Physical is a one-time visit with no membership required. Non-member wound care is $199 for the initial evaluation, then $299/month for up to four scheduled visits — you keep your current primary care doctor.
Membership is available for ages 12 and up. A one-time $299 re-enrollment fee applies if you cancel and later rejoin.

Members who are able can add a small monthly contribution — starting at just $1/month — that goes directly toward subsidizing membership for a neighbor who couldn't otherwise afford care. Every dollar stays right here in our Hillsborough community.

A limited number of memberships are available at a reduced rate for patients facing genuine financial hardship — funded by Pay It Forward contributions and reviewed personally by Dr. Farooqui. No lengthy application, just a conversation.
